What happened

The insight engine tried to create, or revive, an insight while the workspace already has 30 live engine insights (draft or published, not expired, not dismissed or applied).

Why it usually happens

• Earlier runs left many open insights that nobody has reviewed yet.

How to fix

• Do not create more. Refresh an existing insight by calling create_insight with the same subject and action_lever, or stop.
• The limit frees up as insights expire or are dismissed or applied.
